III. Certain Uses and Disclosures Require Your Authorization
We must obtain your written authorization before using or disclosing your PHI for purposes other than those listed above.
Psychotherapy Notes
We maintain psychotherapy notes as defined by HIPAA. These may only be used or disclosed with your authorization unless:

You may restrict disclosure of PHI to a health plan if you paid for services out-of-pocket in full. -

You may request that we contact you in specific ways (e.g., only by mail or phone). -
Inspect and Copy Your PHI
You can request to view or receive a copy of your records. We will respond within 30 days and may charge a reasonable fee. -
Accounting of Disclosures
You may request a list of non-routine disclosures of your PHI. We will respond within 60 days. -
Amend Your PHI
If you believe your records are incorrect or incomplete, you may request an amendment. We may deny the request but will explain the reason in writing. -
Receive a Copy of This Notice
You may request a paper or emailed copy of this Notice at any time.
